Services

Woodstock Primary Care offers comprehensive primary care services by emphasizing health maintenance and disease prevention. We treat common acute illnesses and injuries, as well as diagnose, manage and coordinate the care and rehabilitation of patients with chronic diseases. We advise all patients to take a proactive approach to their care. That means addressing all questions and concerns during the appointment with our physicians. Additionally, patients should contact their health insurance provider to become familiar with their benefits prior to their appointment.

 

Preventative Annual Wellness Exam includes

 

  • Measuring vital signs like blood pressure, BMI, assessment of past medical history and surgical history.

  • Discussion of family history of illness and health conditions, review of patient’s social history.

  • A physical exam performed by a Medical Doctor, an evaluation of current physical activity and goals for improving overall health.

  • Assessment of immunization history, screening for depression and alcohol and substance misuse.

  • Discussion of appropriate cancer screenings.

  • Depending on your risk factors for certain medical conditions and your insurance coverage, additional blood tests may be included as part of this exam.

 

Preventative Annual Wellness Exam does not include evaluation and treatment of any illness or symptoms. Please be aware that any additional tests or screening may not be included as part of an annual wellness exam and may be subject to additional out of pocket costs per your insurance.

 

Acute Care Visit includes

 

  • Evaluation of symptoms related to any acute illness, evaluation of any abnormality detected at annual exam.

  • Referral to specialist for follow-up care (as needed), appropriate diagnostic and lab testing.

  • Prescription medicine reconciliation and refills.

 

Chronic Care Visit includes

 

  • Evaluation of all chronic medical conditions and diagnoses, prescription medicine reconciliation and refills.

  • Evaluation of lifestyle habits including diet, appropriate diagnostic and lab testing.

  • Coordinating care with specialists (as needed), developing follow-up care management plans.

 

 

Here are some tips that will help us give you the best possible care at Woodstock Primary Care:

 

  • Make sure your physician knows all the medications you are taking. This includes prescription and over-the-counter medications, as well as dietary supplements such as vitamins and herbs.

  • Make sure your physician knows about any allergies and adverse reactions you have had to medications. This will help you avoid getting a medication that can harm you.

  • Make sure you know the prescriptions your physician prescribes. Always verify that it is the medication your pharmacy dispenses to you.

  • If you have any questions about the directions on your medication label, ask your pharmacist to explain them to you.

  • Ask for information about possible side effects of medications prescribed to you. If you know what might happen, you will be better prepared if it does, or if something unexpected happens instead.

  • If you see a specialist or another physician outside Woodstock Primary Care, ask for a copy of your medical record and bring it with you the next time you see Woodstock Primary Care physician.

  • If you have tests performed at our office and you do not hear from us within five (5) business days, please call our office.
